How to submit proof of vaccination or apply for a medical or religious exemption

Current employees must upload their vaccination card or exemption form to the Medicat Patient Portal, 哪个是通过登录访问的 MyRIC. All new employees must provide one of the following: proof of vaccination (1st and 2nd dose or J&J), completed medical exemption application, or completed religious exemption application. 

Please Note: any individual that has completed at least one dose may not claim a religious exemption

Proof of vaccination, medical or religious exemption applications must be 电子邮件ed to healthservices@poonamhotel.com. The 健康中心 will review the documentation and follow-up with HR.

Faculty and 工作人员 who have tested positive for COVID-19 or are experiencing symptoms, or who may have had a significant contact with someone that has tested positive for COVID-19 should contact the RIC 人力资源厅 at 401-456-8216 或者完成 COVID-19教职员工报告表. This information is shared directly with Human Resources for appropriate follow-up. 如果你有症状, COVID-19检测结果为阳性, or may have been exposed to someone with COVID-19, please remain off campus until you are cleared by Human Resources to return to campus. Employee Health and Well-Being is a Priority
